Human Factors Associated with the Crash of Korean Airlines Flight 801

An 8 page paper discussing the crash of Flight 801 on Guam in 1997. The
NTSB formally determined that pilot fatigue and inadequate crew training were to blame,
but subsequent investigation revealed that there was a good chance the crash could have
been averted at the minimum safe altitude warning (MSAW) system been operating at the
time of the crash. Previous crashes also had been blamed on pilot error, and a separate
investigation by the FAA resulted in changes to the MSAW system nationwide.

A 10 page contention that aviation maintenance and inspection, like any activity which involves humans, is subject to the limitations of human performance, performance which often as not is replete with errors. Errors, in fact, occur on an everyday basis in aviation. Some errors are minor but others can have disastrous consequences. Errors may be either direct or indirect but they can be minimized by the same analyses as that which is applicable to other human factor-sensitive fields as well.
